Health and Immunization Policy
All students who attend Indiana Wesleyan University’s Marion residential campus are required to create an account on Medproctor using their IWU email address. Students must complete their medical history and TB Screening on this site. If a TB test is indicated, the student will be asked to print a form and upload the test results. Students must also upload a current immunization record. The $10 charge for this site is only for those who want to retrieve their information. For questions or difficulty using Med+Proctor, please use Med+Proctor's 24-hour chat line located on their website.
Necessary documents include:
- TB Test if indicated
- Immunization records including:
- Two MMR (Measles, Mumps, Rubella) vaccines
- Tetanus Pertussis, Diphtheria (T dap) vaccine within the last 10 years
- Record of a meningitis MCV4 given after age 16 and before age 21
- Meningitis B (optional)
- Varicella (optional)
- Hepatitis A (optional)
- Covid-19 Vaccine (strongly recommended)
Proof of immunization may be provided through the following options:
- Completed immunization records signed by a health care provider
- Immunization record forwarded from high school, college, or university
- Copy of a local health department immunization record
- Immunization record maintained by the student or parent (cannot be a baby book record)
Immunization Exemptions
- Medical Necessity: Documentation from a medical provider that states the nature and probable duration of a medical condition or circumstance that contraindicates immunization(s)
- Personal Objection: A signed waiver objecting to the vaccination requirements.
Note: The Health and Immunization Exemption policy does not apply to majors requiring vaccinations. An exemption or objection may impact a student's ability to continue in that major.
In the event of an outbreak of any vaccine-preventable diseases covered by this policy on or near campus, students holding exemptions or objections will be temporarily excluded from all campus activities, classes, residence halls, or the campus of Indiana Wesleyan University at the discretion of the Medical Director and/or the Board of Health until the outbreak is declared to be over.
Makeup of any classes due to such absences is at the discretion of each professor.
International Students
All international students are required to have a test for Tuberculosis (TB) once they arrive in the United States. We require the QuantiFeron Gold TB or the T-Spot test. The test may be obtained at the Wildcat Health Clinic.
Students who do not comply with the Health and Immunization policy will not be allowed to register for the next semester, attend classes, or reside on campus on the first day of that semester until the required information has been submitted to the Wildcat Health Clinic.
These must be received by July 15 of the incoming year for the fall semester, and December 1st for the spring semester. Instructions for Med+Proctor can be found on the Wildcat Health Clinic Portal Page.
